Step 1. Create Survey within SurveyStance Portal (https://portal.surveystance.com)
Step 2. Edit Landing Page
Click 'Add Additional Survey Questions'
Enter the Freshdesk field to capture example: ticket.from_email
Step 3. Copy & Paste Survey into Freshdesk Email Template
Admin -> Email Notifications -> Requestor Notifications
